I released a small update that should technically improve performance a bit, although as a user, one wouldn't notice unless using a really slow storage device. And added a proper line ending for the last line in non-compatibility mode, which shouldn't be used anyway at this time.

What happens if you boot the PSIO without a card inserted? Probably dirt/oxidation on the parallel port pins. Make sure this thing is clean with isopropanol alcohol or something, and try inserting and removing the PSIO 10 times. If that doesn't help, try it another 10 times. If that doesn't help, er...

None that I know of. Also I'm not sure it can be done reasonably, as pre gaps might be different and that information is not in a CU2. Unless you really only need it done for the PSIO, I suggest merging images via a CD drive emulator and imaging software. If the result is identical to what System Co...

It's the other way around, you need to add the leading zero. This should also be a very easy fix for Matt to implement in the upcoming new version of the System Console. Redump's Disc Image Creator also produces IMG files with a second cue sheet for those. I don't know if they're different from CloneCD, but I presume they are not. Did your friend multimeter the switch board install?
What happens when you boot the PSIO without a card?
Make sure the parallel port is clean and then try removing/reinserting the PSIO 10-20 times. There might be corrosion.

What happens when you boot it with PSIO with a card inserted? Any error message? Where does it get stuck? Anything different when you try 1024 KiB allocation size? Does the LED on the PSIO blink?

Without reading up too much on the System Manual: shouldn't it work to put only the 2.6.0 ARM firmware as RECOVERY.BIN on the card, plug it in and give it like 5-10 minutes to flash, then remove RECOVERY.BIN and put the 2.6.5 MENU.SYS on? Hmm, well, do you really need a list for that? It's three modes, one can figure this out per game pretty quickly (trying 3, 1, 2), and once one did, can create the corresponding file to make it the default for that game.
